If you’ve been off sick for more than 7 days
Effective 29th September 2025, we will no longer accept sick note requests via telephone.
To request a sick note, please use one of the following online methods:
- PATCHS Portal: Log in and select the ‘Admin Request’ option. You will be prompted to complete a short questionnaire.
- Practice Website: Complete the Sick Note Request Form available on our website.
Once your request has been submitted:
- If an appointment is available, a Care Navigator will arrange a remote consultation for you.
- If no appointments are currently available, you will be placed on a waiting list and contacted as soon as a slot becomes free.
We appreciate your understanding and support as we work to improve the efficiency of our request handling process.
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
